(ask about our Super Peewee team ages 4-5)

Registration Requirements

Please check our website often for updated information and also refer to the Greater Miami Pop Warner League’s website for the season age levels/teams structure, all Pop Warner required forms, and the season’s weekly games and play-off games schedules… 

 

Registration Requirements

  1. Completed R.P.O.C. Football/Cheer/Dance Registration Form (signed by Parent or Legal guardian)
  2. Completed Pop Warner Player Contract/Parent Consent Form (signed by Parent or Legal guardian)
  3. Completed Official Pop Warner Physical Form (signed by Physician and parent/Legal guardian)
  4. Copy of child’s official Birth Certificate *(Name on all forms must agree with name on the birth certificate)
  5. Copy of previous year – end of school year Report Card *(with all 4 semesters and final grades listed)
  6. Recent Photo for ID Card *(to be taken on site upon registration)
  7. Payment of Full Registration Fees *(on-site payments cash or M.O. only, or on-line registration payment option available)
